Informed Consent, Medical Disclaimer, and Terms of Service
I consent for John-Marco Ariza to provide nutrition, exercise, and health counseling to myself or the client I’m responsible for. The consultation and service processes will provide information, guidance, and support about health factors within my control, such as my physical activity, nutrition, and lifestyle.
I understand that John-Marco Ariza is an MNU certified nutritionist and a certified personal trainer; however, he is not a medically trained doctor, dietitian, or therapist, and does not dispense medical advice, nor will he diagnose or treat any medical condition. John-Marco will provide support and education for an already diagnosed condition where applicable and within his scope of practice. I’m encouraged to seek medical advice if I suspect I have an illness requiring medical attention.
While nutritional, exercise, and health-related support can complement my medical care, I understand these services are not substitutes for medical care. Health evaluation methods made available to me, such as nutrition, exercise, or body composition assessment, are not intended to diagnose disease. Instead, these assessments guide the development of an appropriate health-supportive program and monitor my progress in the service process.
John-Marco Ariza will keep medical records, personal information, and history shared in the service process confidential unless I consent to share such information.
I understand I will set up a one-time or recurring payment to John-Marco Ariza, depending on which service I’m participating in. To end the service process involving a recurring payment, I must provide John-Marco Ariza with a written statement before my upcoming payment, stating that I wish to terminate our collaboration.
I understand that John-Marco Ariza does not offer any guarantees or refunds related to the service process.
I agree to hold John-Marco Ariza harmless for claims or damages concerning our work together. This contract is between John-Marco Ariza and me, and I understand it’s also a release of potential liability.
I have had an opportunity to ask questions about my service, and by accepting, I agree to participation and the above terms. I intend this consent to cover the entire course of my service.
